- SSC Certificate
- B.Sc(Nursing) Provisional/ Original Degree Certificate
- Mark-sheet of B.Sc. (Nursing) for 4 years/ 2 years Post Basic.
- A.P Nursing Council Registration Certificate.
- Study certificate from 4th class to 10th class (in case of private study,
- Original Residence Certificate for the period of study to be submitted).
- Physically Handicapped Certificate i.e., Visually, Hearing and Ortho
- categories (Issued by the Regional Medical Board/ SADAREM) (if applies)
- Caste Certificate issued by the MRO/ Tahsildar concerned.
- Latest EWS Certificate if applicable issued by the Tahsildar. (if applies)
